Group health
How many employees do you need for group health insurance?
There is no legal minimum. Each insurer sets its own, and minimum group sizes are commonly around seven lives, with some insurers going lower. Below that, the honest route is individual cover for your people until the team grows, never a padded group.
We’re eight people. Is that enough for a company health policy, or are we too small?
In short
- There is no legal minimum; insurers set their own, commonly around seven lives.
- Group health is not mandatory for most private companies, but it is the benefit employees value most.
- Below the minimum, use individual policies until the team grows. Never add people who aren’t employees.
- Once eligible, a group policy can cover pre-existing diseases from day one, with no medical checks.
Is there a minimum number of employees for group health insurance?
Not in law. It is an insurer’s underwriting rule. Minimum group sizes are commonly around seven lives, and we have seen insurers accept groups as small as five. Ask whether the minimum counts employees or all covered lives, because insurers differ.
Is group health insurance mandatory for a small business?
For most private companies, no. A temporary direction in 2020 required it, and that direction has lapsed. Separately, ESI is statutory for employees earning up to ₹21,000 a month at establishments with 10 or more employees. Our page on whether group health insurance is mandatory covers this in full.
Can we add people to reach the minimum?
No. A group can only include genuine employees and their covered family members. Padding a group with people who don’t work for you to cross a threshold is misrepresentation, and it puts every claim on the policy at risk, not only theirs. If you are below the minimum, the honest answer is individual cover until the team reaches it.
What can we do below the minimum?
Cover your people individually, and plan the move to group cover. Individual policies for the team, or at least for the people with families or known conditions, give real protection now. Check whether any employees fall under ESI. And when the team reaches the insurer’s minimum, start the move to a group policy well ahead of any planned start date, so nobody is left without cover in between.
Why move to group cover once you can?
| Group policy | Individual policies | |
|---|---|---|
| Pre-existing diseases | Often covered from day one | After a waiting period of up to 36 months |
| Medical checks | Not required to join | Often required, especially with age |
| Family | Spouse, children and parents through the ESC and ESCP tiers | A separate policy or floater for each family |
| Who arranges it | The employer, once a year | Each person, separately |
For a small team, the day-one cover for existing conditions is often the deciding point. It is the one thing an individual policy cannot match.
A five-minute check for a small business
Five questions tell you where you stand:
- How many lives would the policy cover?Count employees and the family members you want to include, and ask the insurer which number it uses.
- Who needs cover first?People with families, older employees and anyone with a known condition.
- Is anyone covered by ESI?Employees earning up to ₹21,000 a month, at establishments with 10 or more employees.
- What individual cover do people already have?Keep it running until the group policy is live.
- When will you cross the minimum?Start the conversation before then, not on the day.

Can a broker help a small business?
